Pathologic is an open world mystery survival exploration action game where you need help from an investigator, a Ripper and a eccentric girl with mystical healing abilities. Explore a plaque-ridden town and solve mysterious murders in an emotional psychological first person survival cult-classic.
Follow my step-by-step guide on installing, configuring and optimizing Pathologic HD in Linux with PlayOnLinux.
Note: This guide applies to the GOG version of Pathologic HD. Other versions may require additional steps.

Mint 19 64-bit
PlayOnLinux: 4.3.4
Wine: 4.0
Wine Installation
Click Tools
Select "Manage Wine Versions"
Look for the Wine Version: 4.0
Select it
Click the arrow pointing to the right
Click Next
Downloading Wine
Extracting
Installed
Click (amd64) Tab
Select 4.0
Click the arrow pointing to the right
Downloading Wine 64-bit
Installed
Wine 4.0 32-bit and 64-bit is installed, you can close this window
PlayOnLinux Setup
Launch PlayOnLinux
Click Install
Click "Install a non-listed program"
Select "Install a program in a new virtual drive"
Click Next
Name the virtual drive: pathologic
Click Next
Check all three options:
Click Next
- Use another version of Wine
- Configure Wine
- Install some libraries
Select Wine 4.0
Click Next
Select "32 bits windows installation"
Click Next
Applications Tab
Windows version: Windows XP
Click Apply
Graphics Tab
Check "Automatically capture the mouse in full-screen windows"
Click OK
PlayOnLinux Packages
Check the following:
Click Next
- POL_Install_corefonts
- POL_Install_d3dcompiler_43
- POL_Install_d3dx9
- POL_Install_devenum
- POL_Install_quartz
- POL_Install_tahoma
- POL_Install_wmp9
- POL_Install_wmp10
Note: All packages should automatically download and install
Installing Pathologic
Click Browse
Select "setup_pathologic_classic_HD_1.0.3 (16813).exe"
Click Open
Click Next
Click OK
Click Options
Check "Yes I have read and accept EULA"
Uncheck "Created desktop icon"
Click Install
Click OK for all errors
Click Exit
PlayOnLinux Shortcut
Select "Game.exe"
Click Next
Name the shortcut: Pathologic
Click Next
Select "I don't want to make another shortcut"
Click Next
PlayOnLinux Configure
Back to PlayOnLinux
Click Configure
General Tab
Wine version: 4.0
Note: Click the + to download other versions of Wine. Click the down-arrow to select other versions of Wine
Display Tab
Video memory size: Enter the amount of memory your video card/chip uses
Installing Media Pack Update (Required to play WMV Videos)
Download this Media Pack Update from Lutris
https://lutris.net/files/tools/windowsmedia-kb942423-x86-intl.exe
Save to your Desktop
Move "windowsmedia-kb942423-x86-int.exe" to the Pathologic Virtual Drive drive_c
Full Path:Wine TabCode:/home/username/.PlayOnLinux/wineprefix/pathologic/drive_c
Click "Command Prompt"
Type "explorer"
Press Enter
Navigate to (C)
Double-Click "windowsmedia-kb942423-x86-intl.exe"
Click Next
Check "I Agree"
Click Next
Click Finish
Close the Command Prompt
Close Configure
Launch Pathologic
Select Pathologic
Click Run
Note: Click debug to see errors and bugs
Optimization
Since this is an HD version of Pathologic all of the textures have been increased in resolution. You may get a bit of frame lagging if the settings are too high
Click Settings
Click Video Tab
Adjust:
Click Apply
- Resolution
- Anisotropic filtering
- Gamma
- Field of View
- V-Sync
- Shadows
- Anti-aliasing
- Depth of Field
- SSAO
- Bloom
- Motion Blur
Conclusion:
Pathologic crashed at every intro screen initially because of the WMV codecs, but after some research I found that we could set Wine to Windows XP, install WMP9 & 10, then install this media update and they play perfectly. Remember this for other games that require WMP codecs. After all of that, Pathologic played perfectly! I was able to save, load and play for over an hour without any issues.
